- Please bring your insurance card with you on your visit
- If you need to make a claim, please provide our staff with a claim form, which you should receive from your insurance carrier
- Our staff can assist you with filling out insurance forms and filing claims
- Please be advised that dental insurance usually does not pay for all of the care that the patient receives, and the patient is responsible for paying their portion of the treatment cost
